Don't you feel like a horrible horendous mom?!?! I would know...it has happened here several times, and so far only four of my six have lost teeth!!!! I am awful.
A funny story, though....my sister is remarried to a man who had never ever been around children before my sister brought her two girls with her to him. Last fall, the little one, who was six, lost her first tooth. Her older sister decided she would play tooth fairy, and she put a dollar bill under her pillow. My bil, though, came in and had a panic attack when he learned she had lost her tooth...and HE ran into there, tripped over a toy, landed IN the bed ON TOP of my 6 year old niece....and she stayed asleep through it all!!!! He then put a TWENTY DOLLAR BILL under her pillow!! Then, before he told my sister, my niece DID wake up, to find $21 from the tooth fairy for her first lost tooth!!!!!! My oldest niece and MY kids all say that THEY all got jipped in their tooth loosing days! Hehehe!!! (And my youngest niece was quite disappointed to learn that the tooth fairy only does that on the FIRST tooth...as after that MOMMY stepped in!!!)

How funny!!! We have also forgotten about the tooth fairy coming and it has taken awhile. Been there, done that!! NOT the $20, but still.... LOL
Answer:

I have done this too! I told him she will stop by when he is at school, since no kids can see her. and sure enough there was money there when he came home from school!
Answer:

I"m SOOOOOOOOOO glad we aren't the only house the tooth fariy has experienced delays with!! Luckily, mine didn't get delayed right in the beginning, not until after my kids were older, and the tooth loss less frequent lol
Answer:

she seems to fly right past mine alot...lol....so now every time dd looses her tooth she sarcastically tells me.....MOM.....YOU WRECKEN THE TOOTHFAIRY WILL REMEMBER TONIGHT...Lol...I tried to get away with putting a dollar under her pillow when she got up to got to the bathroom before when went to sleep...lol...when she got back she readjusted her pillow and found...and she laughed and said....mom......the least you could is wait till I finally go to sleep.....lol...she made me put her tooth back and wait...well...sorry to say....I fell alseep tooooo....lol....so it is not uncommon to go a few days before payment appears...lol
Answer:

OMG, Im so glad its not just me! Have done that several times................. once I think it took several days to almost a week before "she" remembered!!

Problem is I get ready for bed and watch tv in my room not long after the kids do, so by time they fall asleep -- they read in bed -- Im asleep too!

If you're in a snowy location, you can always say that the Tooth Fairy can't fly when it's snowy, or when it's too cold!

I glad to hear that I'm not the only one!! In fact - I think I even forgot the last tooth...It's hard here though...because when they start loosing teeth - it's one right after another...I had my youngest DS loose 3 teeth in 1 week!!!
